This Opportunity:

We are currently looking to fill one Rehabilitation Services Coordinator Full Time Position in the Bridgewater area of Winnipeg.

Starting wage at $28.43 per hour.

Roles and responsibilities:

  • High level support and supervision of clients with brain injury and mental health conditions

  • Supervision and assessment of front-line staff

  • Client Quarterly Report writing and editing

  • Create care plans designed to enhance the efficiency and safe delivery of Community Support Services to clients with various disabilities.

  • Attendance and chairing of various meetings

  • Reviewing and updating client files and binders

  • On-call responsibility

  • Create and maintain goals with clients, and track progress

  • Communicate with client guardians and families

  • Build rapport with community resources to establish connections for clients

Qualifications and experience:

  • 5F driver’s license and vehicle to use at work

  • Clean driver’s abstract

  • Valid Criminal Record Check with the vulnerable persons sector

  • First Aid and CPR C (Intermediate)

  • Ability to travel regularly between MODC programs and the community of Bridgewater

  • Knowledge of cognitive impairment and mental health diagnosis

  • Community based experience working with individuals with a cognitive impairment, mental health condition, or brain injury

  • Strong written and verbal communication skills.

  • Highly organized and excellent time management skills

  • Demonstrated ability to supervise and train employees to include organizing, prioritizing and scheduling work assignments

  • Problem solving and conflict resolution skills

  • Knowledge of human relations

  • Knowledge of behaviour management principles

  • Knowledge of Health & Safety legislation

  • Bachelor’s degree in Human Services program or equivalent preferred

  • 2 – 5 years’ experience in the Social Services/Health Sector, coordinating services for individuals with a disability.

  • General understanding of computer hardware and software

  • Strong ability to assess and determine effective compensatory strategies to support maximum community participation/inclusion.
