Schedule: Full-Time, 1.0 FTE, 80 Hours per 2 weeks, 8 hour day shift

We’re seeking a MSW Case Manager to join our team where you will help to support patients and families during a time of need. You will collaborate with the individual, family, physician, and interdisciplinary teams to coordinate healthcare services to promote optimal clinical, financial, and satisfaction outcomes. We are a tight knit and supportive group. This is an ever-changing job and have the ability to think outside the box.

Qualifications

  • Master's degree of Social Worker required.

Responsibilities

  • Provides counseling and crisis intervention to families and patients.

  • Conducts psychosocial assessments.

  • Serves as a resource to patients and families.

  • Facilitates/ Assists patients and families in open discussions regarding advanced care planning.

  • Being a patient advocate to ensure their needs are met and rights are protected.
