At Home Healthcare PHC/Field Supervisor in Tyler, Texas
At Home Healthcare has aggressively spread into the Austin area after 36 years of providing services in the East Texas area.
The purpose of this position is to coordinate the care of waiver program clients based on an individual service plan established by the Texas Department of Human Services. This position requires having a clean valid driver license as you will be travel to multiple client's homes to initiate their services. You must be able to work Monday-Friday. Eligible for PTO, medical benefits and 401k.
You must either be a Licensed Vocational Nurse or have completed two years of full time study at an accredited college or university. An individual with a high school diploma or GED may substitute one year of full time employment in a supervisory capacity in a health care facility, agency, or community based agency for each required year of college.
Responsible for accurate and timely preparation and processing of DHS required documents; coordinates care of clients and utilizes task planners appropriately in Outlook. Ensures timeliness and completeness of all documentation.
*Responsible for education to providers in client’s home’s to perform DHS assigned tasks and timely preparation of documentation (timesheets.)
*Establishes and maintains rapport with DHS case managers; assist with audits as required. Participates in and attends program IDT meetings as requested by DHS or Administrator. Participates in community relations activities as scheduled.
*Adheres to waiver program requirements.
*Available for on call rotations as required for all waiver programs.
*Provides personal client assistance services in event provider staff are not available.
*Performs supervisory attendant orientation visits to client homes for PHC/FC clients.
*Maintains compliance with all At Home Healthcare policies, procedures, and regulations, including all federal and state laws.
*Makes copies of various documents as required.
*Maintains a clean and safe work environment.
- Completes accurate and timely documentation.
*Accurately prepares and proofreads letters, memos, spreadsheets and other correspondence.
Perform all other duties as requested by supervisor.
A supervisor must be a licensed nurse or have completed two years of full time study at an accredited college or university. Excellent organizational skills and ability to effectively handle multiple tasks required. Professional customer service skills and time management skills required.
