Job Summary
The Community Health Worker will primarily be working out in the community with patients to promote, motivate and improve healthy living. A CHW will assist patients in utilizing resources and accessing services to overcome barriers in order to meet their health goals.
Applicant must be an established resident in the Toms River community.
Responsibilities include:
Meet patients in a clinic or hospital and conduct a needs assessment
Schedule weekly follow-up calls and home visits to patients
Help patients with social issues like homelessness, substance abuse and hunger
Assist patients with organizing their records, making follow-up appointments
Help patients address the social, medical and community problems
Help patients with the tasks of getting medical care, and work on health goals by helping to plan healthy meals or finding time to exercise.
Help patients access resources, including quality of care, childcare services, food, housing, transportation.
