Position Summary

This is a fulltime remote role with 50-75% travel required for face-to-face visits with members in Seminole, Orange, Brevard and Osceola Counties. Schedule is Monday-Friday, 8:00am-5:00pm, standard business hours.

We are seeking self-motivated, energetic, detail oriented, highly organized, tech-savvy Clinical Case Management to join our Case Management team. This opportunity offers a competitive salary and full benefits. Clinical Case Management is responsible for telephonically and/or face to face assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member's overall wellness. Clinical Case Management will effectively manage a caseload that includes supportive and medically complex members. Develops a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member's overall wellness through integration. Clinical Case Management will determine appropriate services and supports due to member's health needs; including but not limited to: Prior Authorizations, Coordination with PCP and skilled providers, Condition management information, Medication review, Community resources and supports.

Required Qualifications

  • Active and unrestricted RN license in FL

  • Bi-lingual in English and Spanish

  • Must reside in Orange County or an adjacent county

  • Willing and able to travel 50-75% of their time to meet members face to face in Seminole, Orange, Brevard and Osceola Counties

  • 1+ year computer proficiency in Microsoft Word, Excel, and Outlook

  • Experience in behavioral health or a long-term care setting

  • Case management and discharge planning experience

  • Managed Care experience

  • Ability to multitask, prioritize and effectively adapt to a fast-paced changing environment

  • Effective communication skills, both verbal and written

Education

  • Associates Degree in Nursing required

  • Bachelors Degree in Nursing preferred

Pay Range

The typical pay range for this role is:

$54,095.60 - $116,760.80

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
