Must be currently enrolled in a PharmD Program. Must be 3rd or 4th year pharmacy intern. Must have a minimum GPA of 3.0 and an active NYS Pharmacy Intern Permit.

Internship Description:

SUNY Stony Brook WTCHP is a Long Island Clinical Center of Excellence that provides exceptional clinical care to 9/11 responders. An integrative approach is emphasized to deliver personalized, patient-centered care. This is achieved by incorporating preventative care and treatment, social work, psychiatry, pharmacy and other specialties into the clinical practice model. Our clinic, located in Commack, NY, is seeking one intern to provide outstanding pharmacy-related care to our patients.

  • Under the direct supervision of a pharmacist, the Pharmacy Intern may perform all duties of a licensed pharmacist.

  • Accurately performing a medication reconciliation for patients in the clinic, which includes entry of medication history into the electronic medical record. The patient's' home medication list will be obtained and clarified to ensure accuracy of all prescription, over-the-counter medications, and supplements.

  • Obtaining medication histories in an accurate, professional and efficient manner for patients. Contacts community pharmacies, other providers, and patient family members to obtain needed medication information.

  • Conducts face to face interviews with patients about their current medication use.

  • Interacts with and provides medication history and other related patient information to nurses, physicians, pharmacists and other medical professionals as appropriate.

  • Actively participates in ongoing quality improvement programs focused on medication reconciliation.

  • Interviewing patients and/or family members regarding medication allergies and documenting the allergies appropriately.

  • Counseling patients on all of their medications, vitamins, and supplements.

  • Reviewing patient profiles to identify medication-related issues, such as drug-drug interactions, and communicating such information with the clinical pharmacist(s) and providers.

  • Rudimentary research assistance coordinated by and under the supervision of the clinical pharmacists. Other duties or projects as assigned as appropriate to rank and departmental mission.
