Responsibilities

The RN Care Coordinator assists the Nurse Manager with the timely and accurate completion of both the required assessments from admission through discharge. Specific job responsibilities include: The RN Care Coordinator applies the nursing process through assessments which gather the individualized resident data, followed by decision-making and care planning based on resident strengths and weaknesses, implementation, and evaluation with the goal for the resident to function at maximum levels. The highly specialized RN Care Coordinator will ensure the completion of the required assessments accurately and efficiently through well-developed organizational skills, leadership, and a thorough understanding of the regulations. The RN Care Coordinator can be flexible with the scheduling needs of the unit at large, assisting per direction of the Nurse Manager, while focusing on timely completion of the assessments in compliance with government standards and regulations.. The RN Care Coordinator must utilize their understanding of the regulations and software functions to accurately and efficiently develop care plans for the residents at Charter House.. The RN Care Coordinator will assist with coordination of care, services, and treatments with the interdisciplinary team on all care venues to meet the ongoing and changing needs of the client. The Care Coordinator can be flexible with the scheduling needs of the unit to work as staff RN for the unit. The Wound Management function is necessary to provide consistent oversight and collaboration to optimize wound healing. The RN Care Coordinator will collaborate with the Physician, resident family members, Nurse Manager and Health Center Staff.

Qualifications

Requires a nursing degree from an accredited college or university, a current Minnesota RN license, and a minimum of one year of experience as a licensed nurse in a hospital, long-term facility, or other related health care facility. Prior experience in MDS coordination and MDS Certification is preferred. Must have a good understanding of regulations related to work area. Must be able to work independently. Requires good communication and human relation skills to interact with Health Service and Charter House staff. Strong problem solving skills are necessary with the ability to collect, compare, sort and prioritize information to be used in the decision-making process for groups of resident and employees. Must be able to adapt to unpredictable situations and stress. Works flexible hours which may include days, evenings, nights, holidays and weekends. MN Registered Nurse License CPR certification

Experience and tenure may be considered along with internal equity when job offers are extended.; $33.09 - $48.47 / hour
