Position Summary:

This is a full time, 40-hour per week remote position for a Registered Dietitian. Work at Home from any state; M-F (8:30-5:00 pm in your time zone); one late shift (11:30-8:00 pm in your time zone) per week. Experience working with patients with diabetes.

Job Purpose and Summary:

Be a part of shaping the health of Caremark members with diabetes as part of the exciting, new Nutrition coaching team within the Transform Diabetes Care Program. Within this role, you will be responsible for using a collaborative process of assessment, planning, implementation and evaluation to engage, educate, coach/promote members in decisions related to achieving and maintaining optimal nutrition status and management of their diabetes and/or other conditions (e.g. CKD, CVD). You will motivate and engage members telephonically and via live messaging over the course of multiple coaching sessions and provide them with personalized coaching and education to help them meet their goals and encourage positive behavior change.

Primary Job Duties & Responsibilities:

  • Assessment of members through the use of clinical tools and information/data review, uses knowledge and experience of nutrition and diet planning to conduct comprehensive evaluation of member’s nutritional status and risks. Utilizes assessment techniques to determine member’s level of health literacy, technology capabilities/access and readiness to change

  • Leverages health/nutritional assessment data to establish a recommended nutritional approach based on member’s health status/risks, education, preferences, and support needs

  • Identifies and escalates quality of care issues through established channels

  • Interprets and utilizes clinical guidelines/criteria to positively impact member’s health

  • Provides up-to-date information to help facilitate the member’s understanding of their health status

  • Develops and monitors established nutrition plans in collaboration with the member to meet the member’s goals

  • Demonstrated ability to communicate, problem solve, work effectively with people, and use good judgement in challenging situations

  • Utilizes interviewing and active listening skills, provides nutritional coaching, information and support to empower members to make ongoing independent healthy lifestyle choices

  • Ability to influence/motivate members and promote positive behavior change through a mix of intervention types including telephonic and live messaging

Required Qualifications:

  • Registered Dietitian (RD) certification from Commission on Dietetic Registration.

  • 3+ years experience with diabetes management

  • 3 + years of proficiency with Microsoft Office application: Word, Outlook, Excel, Teams and Power Point experience

  • 3+ years of experience with the ability to deliver health education coaching via telephone 1:1 and via video chat.

  • Must hold an unrestricted RD license in state of residence and have the ability to be licensed in additional states.

  • 3+ years Experience in related areas such as weight management, nutrition or healthy lifestyle behavior change

  • 3+ years experience collaborating and coordinating care with providers and other clinicians

Preferred Qualifications:

  • Multiple state licensure

  • Certified Diabetes Care and Education Specialist (CDCES)

  • Previous telephonic coaching experience

  • Virtual coaching experience via webcam

  • Experienced in Epic EMR data base functionality and documentation

  • Ability to work independently as well as part of a group

  • Excellent communication skills and must be comfortable with public speaking

Education:

Bachelor of Science in Nutrition, Dietetics or related field required

Master’s in Nutrition, Dietetics, or related field preferred

Pay Range

The typical pay range for this role is:

$43,888.00 - $91,052.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
