CVS Health Health Coach Dietitian in Phoenix, Arizona

The Registered Dietitian is part of the Aetna One Advocacy (A1A) and may require some in office training and ongoing work in Chandler, Arizona. There are requirements to live within a commutable distance (typically within 45 minutes maximum).

Normal Working Schedule: Monday through Friday 8 am -4:30 in zone of residence with flexibility needed for member calls after 4:30 pm.

Registered Dietitian Role:

  • Utilizes a collaborative process of assessment, planning, implementation, and evaluation, to engage, educate, and promote/influence members in decisions related to achieving and maintaining optimal nutritional status.

    Assessment of Members:

  • Through the use of clinical tools and information/data review, uses knowledge and experience of nutrition and diet planning to conduct comprehensive evaluation of member's nutritional status and risks.

    • Utilizes assessment techniques to determine member’s level of health literacy, technology capabilities/access, and readiness to change.

    Enhancement of Medical Appropriateness and Quality of Care:

  • By assimilating health/nutritional assessment data analysis, establishes a plan of care based on member’s health status/risks, education, and support needs.

  • Partners with members to identify opportunities for integration into available internal/external programs.

  • Using holistic approach consults with supervisors or others to overcome barriers with assisting members in meeting goals and objectives.

  • Identifies and escalates quality of care issues through established channels.

  • Utilizes influencing/motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes to achieve optimum level of health.

    -Utilizing interviewing and active listening skills, provides nutritional coaching, information, and support to empower the member to make ongoing independent healthy lifestyle choices.

  • Interprets and utilizes clinical guidelines/criteria to positively impact members health.

    -Provides up-to-date information to help facilitate the member’s understanding of his/her health status.

    -Helps member actively and knowledgably participate with their provider in healthcare decision-making.

    Monitoring, Evaluation and Documentation of Care:

    -Develops and monitors established nutrition plans, in collaboration with the member and/or attending physician, to meet the member’s goals.

    -Utilizes internal policy and procedure in compliance with regulatory and accreditation guidelines.

    Required Qualifications:

  • Registered Dietitian through the Commission on Dietetic Registration required

  • Must live within a commutable distance to Chandler, Arizona (typically within 45 minutes maximum).

  • Must be willing to represent A1A team onsite as guided by supervisor for plan sponsor activities and educational needs

-2+ years demonstrated experience with personal computer, keyboard navigation, and MS Office Suite applications (Teams, Outlook, Word, Excel, SharePoint, etc.)

  • 5+ years’ experience in a variety of clinical areas such as medical nutrition therapy, disease management and health lifestyle behavior change

  • 5+ years of coaching experience and working on a multidisciplinary team

  • Must obtain licensure in multiple states where members reside, with CVS supporting the cost of required licensure

    Preferred Qualifications:

  • Registered Dietitian Licensure required in state of residence if applicable

  • Certified Diabetes Care and Education Specialist (CDCES)

  • Experience in facilitating patient/client positive behavioral change

  • Managed Care Organization experience preferred

  • Ability to work in multiple systems and in a fast-paced environment

  • Demonstrated ability to communicate, problem solve, and work effectively with people, use good judgm ent in challenging situations

  • Experience in facilitating patient/client positive behavioral change. Background in motivational interviewing, active listening skills, Motivational Interviewing, mindfulness based, and other evidence-based coaching techniques preferred.

    Education:

  • Bachelor’s degree required

Pay Range

The typical pay range for this role is:

$46,597.20 - $101,200.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
