Transitions Coordinator Title: Transitions Coordinator Location: Lower East Side Org Unit: GIM Clinical Operations Support Work Days: Weekly Hours: 35.00 Exemption Status: Non-Exempt Salary Range: $27.61 - $34.45 *As required under NYC Human Rights Law Int 1208-2018 - Salary range for this role when Hired for NYC Offices Position Summary Under supervision, supports the optimal continuum of patient care by facilitating communications with Hospitalists and primary care physicians, securing appointments at discharge, and notifying community caregivers of test and lab results post-discharge Job Responsibilities Meets with Hospitalist Teaching Teams and Hospitalist PA Teams to identify patient admissions, transfers, discharges and daily plans of care. Secures daily patient discharge list and books follow-up post discharge appointments with primary care physicians. Forwards discharge summaries to primary care providers. Identifies pending lab/tests and forwards results to outpatient physician when complete. Maintains logs of activity. Education High School Diploma Experience Approximately 1 year of prior related experience Knowledge, Skills and Abilities Demonstrated experience in communicating, and developing relationships, with clinical staff and ancillary services. Previous experience working with eMR systems.
