The above hiring range represents the University's good faith and reasonable estimate of the range of possible compensation at the time of posting. Position Summary The Clinical Coordinator III (RN) will work in partnership with an interventional cardiologist in Columbia Interventional Cardiovascular Care. The RN will see patients followed by the renal transplant service initially with the MD and eventually independently when the RN becomes a full NP and prescreen them for cardiovascular disease. Additionally, this position will support peer-to-peer insurance authorizations, prescription refills, urgent medical calls, and provide results to patients who have had diagnostic testing. Responsibilities Prescreen patients awaiting a kidney transplant for any cardiovascular disease. Based on a cardio-renal algorithm, refer for follow up testing and evaluation by a physician Round on inpatient renal transplant patients Educate patients and their families on conditions and treatment options including risks Coordinate testing and obtain authorizations Respond to patient and family inquiries Participate in weekly coordination of care meetings Enter data into a research database Assist in patient evaluations with cardiologists involved in the renal transplant program See post implant patients in follow up appointments Serve as a liaison between the cardiovascular and renal programs Electronically order new prescriptions as well as prescription refills Perform other duties as required Minimum Qualifications Requires a Bachelor's of Science degree in Nursing (BSN), plus four years of related experience or equivalent in education, training and experience. NYS Licensure as a Registered Nurse. At least two years of experience in a hospital setting, preferably in cardiology. Preferred Qualifications Bilingual (Spanish) preferred but not required. Other Requirements The candidate must be able to work collaboratively within a diverse team of professionals and must be a highly organized. The candidate must have strong clinical skills with the ability to make clinical decisions on care pathways for each patient. The candidate must have excellent communication skills and be able to work individually and as part of a team. The candidate must acknowledge that the patient is our highest priority. Must be able to manage multiple cross-functional communications and process in a timely manner. Must have a high degree of professionalism in all communications. Experience in managing complex schedules. Participation in Medical Surveillance Program. Must successfully meet systems training requirements.
