  1. In support of the overall mission, plans, directs and oversees programmatic activities. 2. Develops, implements, and maintains operational policies and procedures. 3. Manages program communications; identifies outreach potential, and promotes philanthropic and collaborative support of the program. 4. Works closely with leadership, internal and external colleagues, and community residents to further develop and implement strategic plans. 5. Identifies, solicits, and cultivates community partnerships and collaborations to assist in the development and growth of the program. 6. Tracks all program activities and regularly informs leadership of progress on each initiative. Recommends structural or programmatic adjustments, changes or additions based on full knowledge of missions, goals, and objectives. Develops metrics to identify and measure the success of the program. 7. Assists in identifying and evaluating potential future funding sources, and contributes to the submission of grants and contracts supporting the program, including the annual submission and writing of all content areas and budgetary sections of grant applications. 8. Develops and manages the program's operating budget. 9. Manages the administrative infrastructure of the program. Manages human resources including staffing and hiring, supervision, performance development, counseling and discipline, if warranted. Oversees and manages information systems, facilities, and space needs. 10. May perform other duties as assigned. Required Skill/ability 1: Strong ability to use empathetic probing skills to identify and address unique patient needs within family & cultural structures. Proven ability to rapidly engage with multiple patients throughout the workday while effectively meeting the needs of patients and program objectives. Required Skill/ability 2: Ability to effectively communicate with a broad range of multidisciplinary stakeholders (i.e., patients, families, various medical providers, care network leadership, etc.). Proven ability to provide clear communication and critical information to internal team and external stakeholders and elicit feedback. Required Skill/ability 3: Ability to consistently manage a patient caseload by developing and implementing strong organizational and time management skills that are flexible to rapidly changing priorities. Proven ability to schedule and coordinate program activities, develop action items, and use other strategies to keep work moving according to timeline. Required Skill/ability 4: Ability to independently assess and initiate action in real time using sound judgment and patient-centered advocacy at both program and patient levels. Ability to consistently develop and utilize program-related documents, and other innovative solutions to reduce patient barriers. Required Skill/ability 5: Strong analytical skills. Proven ability to independently evaluate and assess patients' suitability for inclusion into program, determine patient eligibility, and coordinate enrollment. Demonstrated ability to develop, submit, and present reports based on patient and program level data to program leadership. Preferred Education: Demonstrated experience in an oncology healthcare setting. Proven experience working with the EPIC Electronic Medical Record, health education, and patient & provider face-to-face communication are highly desirable. Bilingual Spanish (or multilingual). Demonstrated experience working with community outreach or community-based organizations/initiatives.
