We Are Hiring

Population Health Coordinator - Value Based Care - Full-Time, Day - 20th Avenue Bldg - Nashville, TN

Full-Time days - 80% remote work. Must be willing to drive into Nashville at least once a week (after COVID).

Medical Assistant or Licensed Practical Nurse credentialing preferred. Primary Care Physician Office and/or Population Health experience strongly preferred.

What You Will Do

As an associate with Saint Thomas Health Svcs-TN, you will have the opportunity to coordinate and identify care needs (gaps in care) for patients attributed to a primary care practice.

Responsibilities:

  • Identifies patients through the health plans gaps in care list, state immunization databases, the patient medical record and the use of a patient disease management registry.

  • Collaborates with the providers and reviews care needs and validates these needs against the medical record.

  • Reaches out to patients with validated gaps in care and facilitate meeting these needs.

  • Supports quality outcomes and high reliability in the Patient Centered Medical Home.

What You Will Need

Licenses/Certifications/Registration:

  • Preferred Credential(s):

  • BLS Provider.

Education:

  • Diploma

  • High school diploma or GED required.

Work Experience:

  • None required.
