Atrium Health Rehab Therapist Team Lead - Atrium Health at Home Union - FT in Monroe, North Carolina

Job Summary

Primary Purpose In addition to the provision of home health therapy services as directed by Branch policy & procedure, Medicare Conditions of Participation, State/Federal Regulations, the Therapy Team Leader will act as the clinical resource for the branch therapists including new hires and students. Assumes a leadership role ensuring clinical standards of practice. Coordinates activities to maintain and improve the quality of patient care delivered while maintaining a direct patient care caseload of 50%.

Major Responsibilities

·Assists the Branch/Executive Director and Clinical Operations Manager with staff compliance with policies and procedures, documentation, and other related departmental issues.

· May complete or assist Clinical Services Supervisor and Clinical Operations Manager with performance reviews by offering performance feedback for assigned staff.

· Assists Clinical Operations Manager with staffing schedules and department organization.

· Assures staff and program compliance with established standards, practices, and regulations.

· Participates in performance improvement activities and communicates information that enables root cause analysis of data to identify opportunities for improvement.

· Maintains/enhances clinical practice skills by attending, participating in/conducting staff development/continuing education programs.

· Participates or leads training and in-service programs for other agency staff as directed.

· Assists Clinical Operations Manager with orientation as preceptor for new hires.

· Acts as the Center Coordinator of Clinical Education (CCCE) for the branch.

· Supports clinical staff in the development and implementation of the plan of care, coordination of care delivery and strategies to promote health outcomes and patient safety.

· Responsible for the daily management of an assigned patient caseload, ensuring timely care and documentation to meet the patent needs/services as prescribed in the established plan of care and per MD orders.

· Develops and provides oversight of a plan of care that is effectively communicated with the parties involved and supports quality patient outcomes and patient satisfaction.

· Implements the identified plan and directs the care coordination of a multidisciplinary clinical team approach, supervision of discipline specific therapy assistant and/or HHAs involved in the care of the assigned patient caseload to promote health and safe outcomes.

· Performs ordered treatments, therapies, and/or procedures in accordance with State Practice Act and Home Health Conditions of Participation, JCAHO and agency policy and procedures.

· Communicates and works with the care team to coordinate patients' schedule and treatment programs.

· Promotes shared governance and decision-making activities, developing and nurturing research to positively affect clinical outcomes and promotion.

Leadership and OASIS experience preferred.

Minimum Job Requirements

Education

Bachelor’s degree from an accredited discipline specific therapy program in Physical Therapy or Occupational Therapy, or Master’s degree in Speech Language Pathology required.

Certification / License

  • Must always have a current state driver’s license and auto insurance.

  • Bachelor’s degree in Physical Therapy or Occupational Therapy or Master’s degree in Speech Language Pathology.

  • Maintains active membership in discipline specific professional organization (APTA, AOTA or ASHA).

  • Basic Life Support for Healthcare Provider (BLS HCP) from AHA required.

  • Expected to obtain OASIS Certification within one year of hire date.

Work Experience

One year experience as a licensed therapist in a clinical care setting

Knowledge / Skills / Abilities

Personal vehicles must be well maintained and in safe driving condition.

Physical Requirements and Working Conditions

Works in patients' homes in various conditions. Possible exposure to blood and bodily fluids and infectious diseases. Ability to work flexible schedule to meet patients' needs. Ability to travel extensively. Some exposure to unpleasant weather. Ability to communicate verbally and in writing with multidisciplinary health team. Work requires walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling. Must be able to lift and support the weight of 50 pounds in handling patients, medical equipment, and supplies. Must speak English in good, understandable terms. Intact sense of sight, hearing, smell, touch. Finger dexterity. Critical thinking and ability to concentrate. Must be able to respond quickly to changes in patient conditions.
