Our highly-trained providers are your partners for improved health. Our primary care services are available to adults 18 and over and include wellness exams, treatment of minor illnesses and injuries, and management of chronic conditions such as diabetes and high blood pressure. When more specialized care is needed, you can have peace of mind knowing your provider is connected to the world-class specialists within the Atrium Health network.

Clinic Hours: Mon – Fri, 8am – 5pm
Outpatient Only
No Nights/No Weekends
Minimal Call (Phone Only): Shared Amongst Providers in Group
Ideal candidate:
Passion for chronic care management
Team player who is interested in being part of a collegial environment focused on efficiency and quality for our adult patient population
Ability to work independently and manage various aspects of electronic documentation during the patient care process
