Eastern Connecticut Health Network Community Health Worker/Lifestyle Coach in Manchester, Connecticut
The Community Health Worker (CHW) provides community support and engagement for patients within the ECHN service area that would benefit from the ECHN Diabetes Prevention Program in order to help improve health outcomes for patients with pre-diabetes or diabetes. The CHW will provide care coordination services to address health and social needs of identified patients. The Community Health Worker will be responsible to reach priority populations, build trust and improve retention of participants of the Diabetes Prevention Program. The CHW will be responsible to recruit participants, and cultivate relationships with health care providers, local churches, community members, etc. and promote awareness and knowledge of diabetes and education around prevention. The CHW will also be responsible for the timely completion of brief process evaluation forms after core recruitment activities. In addition, the CHW will be trained to work as a lifestyle coach to deliver the Diabetes Prevention Program as needed based on volume/community demand. The CHW is responsible, along with other members of Diabetes Program, for contributing to the clinical, quality, financial and patient satisfaction outcomes.
High School Diploma or equivalent require, Associates degree in Health or Human Services preferred. EXPERIENCE:
Minimum of 1 year experience in a community/outpatient setting is preferred.
Excellent interpersonal verbal and written and communications skills with patients from all age groups.
Must have access to reliable transportation to travel between multiple work sites as travel is required to perform the essential functions of the position.
Must be able to speak, read, and write using the English language.

Works under the direction of a Diabetes Program Coordinator in determining priority populations, and identifying areas for community and provider engagement in the development and support of the Diabetes department and the Diabetes Prevention Program.
Engages with priority populations and recruits participants for the Diabetes Prevention Program, promoting awareness and knowledge of diabetes and education around prevention.
Engages and builds relationships of trust with health care providers, community businesses, social service agencies, etc. in promoting diabetes education, prevention and recruitment in the diabetes prevention program.
Acts as a support for enrolled participants which includes advocacy and in improving retention of the participants in their class.
Attend and contributes to Diabetes Staff Meetings.
Work as a lifestyle coach in delivering the Diabetes Prevention Program with adherence to a CDC approved curriculum with the required intensity and duration to class participants in an effective, meaningful and completing way. Will be required to participate in lifestyle coach training and other continuing education opportunities in order to maintain competence as a lifestyle coach.
Timely and accurate completion of process evaluation forms, classroom documentation and other documentation forms as necessary.
Attends relevant training as necessary to maintain knowledge in the field of diabetes and diabetes prevention.
