CVS Health Case Management Coord in Louisville, Kentucky
The goal of the Start Strong Re-Entry program is to provide individuals who are being released from incarceration with additional resources and support, as they transition back to their lives and communities, with the overall goal of being a reduction in recidivism. We utilize a collaborative approach with members to increase their success with their re-entry plan. Whether the member has physical health and/or behavioral health needs, our Case Management Coordinator will work to ensure the re-entry plan accounts for both.
- 1+ years of Case Management experience- Criminal justice experience- Experience working with individuals who are incarcerated and/or leaving facilities- Experience connecting individuals to resources and supports related to Social Determinants of Health and substance use disorders- Post COVID restrictions there will be local travel within Kentucky. Travel could include meeting members face to face in the community and participating in meetings with community-based organizations. - Bachelor's Degree required
COVID-19 Vaccination Requirement

As a part of this process, you will be required to provide information or documentation about the reason you cannot be vaccinated.
