Responsible of all ascertainment of medical outcomes in Hispanic Community Health Study/Study of Latinos (HCHS/SOL) Responsibilities include: Record Acquisition and Management Contacting participants or their proxies to get consents and medical releases. Re-establish contact with participants lost to follow up by telephone contacts & update personal contact information Identification, investigation and documentation of potential outcomes (cancer, heart attack, stroke, osteoporosis, fractures and other conditions. Requires familiarity with medical aspects of conditions, with tests, procedures used and sound judgment in preparing documentation for adjudication by third party physicians as to the nature of outcomes. Acquisition of death certificate, hospital summary, autopsy report & narrative history form primary health physician & submission to Clinical Coordinating Center, IRB & adjudicators. Retrieval of participant charts to & from off-site facility. Route requested outcomes adjudication case packets for central adjudication Organize and manage both paper and electronic medical records packets of study participants, including redaction of identifiers Data Management and Quality Assurance Manage batch electronic medical records of study participants and maintain linked databases using data management tools like Excel, Access. Knowledge of data analysis software such as SAS is a plus but not required. Strong skills in data management, including performing quality assurance, developing data dictionaries, routine updates are a plus Query on-line databases to obtain and abstract participant medical record information Run reports leading to potential outcomes ascertainment. Data entry of outcomes study forms & analysis Monitor, track timeliness & completeness of documents requested from external sources. Maintain confidentiality of research participants' identifying information and medical histories Collaboration Obtaining cooperation from medical records staff in over 126 hospitals and over 1,000 providers to obtain and abstract hospital and physician records, pathology, radiology and other lab reports in order to compile multiple sources into case-packets to be adjudicated by physician-adjudicators. Member of the national NIH committee that is developing procedures and protocol for outcomes ascertainment in the HCHS/SOL cohort. Responsible for tracking and follow-up and liaison with NIH and the study coordinating centers in Seattle and North Carolina. Responsible for collaboration with the coordinating center to review adjudicated cases packets Administrative and Staff Supervision Oversight of outcomes support staff in identification, investigation & documentation of study endpoints. Responsible for training and overseeing the work of outcomes assistant. Also to assist in hiring new personnel. Participate in regular conference calls. Interpret & implement protocol policies & procedures Creativity, enthusiasm, and excellent communication skills to interact with multiple parties (researchers, clinicians, informatics staff).
