Atrium Health RN Patient Flow Coordinator - Atrium Health Hospitalists Services PT Nights in Charlotte, North Carolina

Job Summary

Coordinates the appropriate placement of patients through professional judgment, communication, and defined organizational standards and policy.

Essential Functions

  • Assesses patients' needs upon admission and utilizes the nursing process in providing responsible patient care.

  • Documents all significant information on appropriate patient record.

  • Communicates significant information and collaborates with teammates responsible for, or involved in, the care of the patient.

  • Anticipates changing patient care needs and department situations.

  • Resolves patient related problems within scope of responsibility.

  • Participates in the facility's Performance Improvement program.

  • Follows organizational policy and procedure and department protocols and serves as a resource for others.

  • Demonstrates age-specific competency in clinical practice environment.

Physical Requirements

General hospital environment with frequent exposure to chemicals, pathogens and patient blood and body fluids. Environment is climate controlled. Frequent lifting, pulling, pushing and carrying. Frequent walking, standing, stooping and reaching.

Education, Experience and Certifications

Graduate from an accredited School of Nursing required; Bachelor's Degree preferred. License to practice as Registered Nurse or temporary Registered Nurse petitioner in the state of North Carolina required. Basic Life Support for Healthcare Provider (BLS HCP) from AHA required. Current successful completion of ACLS from AHA required. High-level of problem solving to identify solutions with human, technical, clinical or administrative problems.
