Atrium Health Program Coordinator - Atrium Health Infectious Disease FT in Charlotte, North Carolina

Job Summary

Responsible for coordinating with the medical team and other disciplines associated with transition to facilitate optimal health outcomes and successful, seamless transitions along the healthcare continuum. The Coordinator uses critical thinking, interpersonal skills, communication, as well as assessment tools to determine community resource service needs.

Essential Functions

  • Functions as a liaison for patient/family in navigating the continuum of care.

  • Assesses, plans, coordinates, and evaluates services for client needs with the goal of equipping and empowering individuals and their families to easily access resources and adopt healthy lifestyles that will increase their ability to remain healthy.

  • Works closely with clinics and agency personnel to ensure patient care is seamless, efficient, effective and appropriate to the individual.

  • Acts as a resource to Physicians and Administration.

  • Completes accurate and appropriate documentation, logs, and/or patient forms and records to ensure compliance with regulations, agency policy, payer requirements, and standards of care.

  • Establishes lines of communication among appropriate agencies and personnel to ensure flow of information and provide a continuum of care, based upon client needs.

  • Maintains statistics and prepares and submits agency and client activity reports as required and/or requested.

  • Continually improves knowledge of community resources, service providers, and coverage issues through individual reading and research, attendance at professional organization meetings, in-services, and community workshops.

  • Ensures that Established productivity standards are met and that resources are utilized efficiently and effectively.

Physical Requirements

Work requires walking, standing, sitting, lifting, reaching, bending, and stooping. Must lift a minimum of 35 pounds shoulder high. Must speak English in effective, comprehensible terms. Ability to communicate verbally and in writing. Spanish proficiency a plus. Must have intact sense of sight and hearing, finger dexterity, critical thinking, and ability to concentrate. Must be able to respond quickly to changes in assignments. Ability to travel between facilities.

Education, Experience and Certifications.

Bachelor’s Degree in Social Work or 5 years of equivalent work experience required.
