This is a full-time position for the Lifestyle Medicine at Home (LMatHome) program. The APP will work closely with the supervising physician, primary care providers, peer support specialists, and other healthcare professionals to deliver comprehensive, patient-centered care that emphasizes health promotion, disease prevention, and sustainable lifestyle changes.
Lifestyle Medicine at Home is an evidence-based program that provides personalized, in-home medical consultations to families seeking to prevent, treat, and reverse chronic diseases. Delivered in-person by a board-certified lifestyle medicine provider and an interprofessional team including peer support specialists, LMatHome meets families where they are and collaborates to create a personalized action plan. The Lifestyle Medicine Action Plan includes personalized lifestyle education, goal setting, community referrals, and lifestyle equipment.
The APP will engage with families from diverse socioeconomic backgrounds, conducting home visits to assess lifestyle needs and develop personalized lifestyle medicine action plans. In this role, the APP will facilitate family education and oversee follow-ups through a variety of lifestyle medicine services. The APP is supervised by the Lifestyle Medicine at Home medical director.
This position requires expertise in motivational interviewing, lifestyle medicine, and person-centered care, with a strong emphasis on addressing the unique needs of patients from various socioeconomic backgrounds.
Position Highlights:
Conduct virtual and home visits to assess the biological, psychological and sociological needs of patients and families.
Co-create family-centered, personalized lifestyle medicine action plans, including high quality lifestyle education, referrals to programs and agencies, and recommendations for lifestyle equipment.
Utilize motivational interviewing techniques and technology to engage patients and families in adopting and maintaining healthy behaviors
Coordinate care with LMatHome staff, outpatient, and inpatient healthcare team members to ensure comprehensive, continuous, and patient-centered care.
Plan and leads recurring educational events and group visits for patients and caregivers.
Participate in community outreach including media and community events
Provide virtual follow-up care, including interprofessional chronic care management review and remote patient monitoring
Assist in the review and development of clinical and operational policies and procedures in the specialty of lifestyle medicine.

NC and SC Licensure and DEA
American Heart Association BLS
Must be able to work autonomously
