Position Summary

  • Relationship building with key external stakeholders to better understand service system drivers and barriers/constraints from different perspectives.

  • Partnering with and convening external stakeholders to develop and implement system-wide solutions that will focus on:

  • Adult service delivery system consistent with DHHR plan to identify gaps in care and needed changes using public data and information from the health plan

  • Community support programs that ensure the needs of adults and families are met with an emphasis on transitioning aging out youth to independent living

  • Assist Care Advovate Team colleagues and other Health Plan managers in various projects, work groups with adult/child welfare agencies, RTCs, SUD providers, and the justice system

  • Collaborates with adult welfare and other adult serving agencies to ensure coordination of efforts

  • Collaboratively participate in rounds

  • Collaborate with local, state, and national partners and stakeholders

  • Provides training and support in effort to provide continuous improvement in serving adults and families

  • Establishes a positive organization and company image with the state health care community

Functions/Role:

  • CAT Team - participates in the development of a comprehensive network of statewide, community, and plan based services that reduces barriers, improves access, and identifies and solves for systemic process challenges to mee the needs of members and their circle of support.

  • Drives collaboration across agencies and community-based organizations to solve barriers and challenges.

  • Builds meaningful partnerships and takes part in solution identification with providers, state and local agencies, and stakeholders.

  • Identifies gaps and potential systemic solutions that address target populations.

  • Serves in a cross-functional capacity collaborating with multiple departments, acting as a conduit for both internal and external communication.

  • Adult Administrator - Collaborates with adult welfare, adult corrections, and other adult serving agencies. Also responsible for:

  • Conducts market surveys to identify system gaps; collaborating with health plan staff including value-based solutions, providers, and stakeholders to mitigate gaps.

  • Develops and implements community support programs and processes to ensure we meet the needs of adults and families. An example WV Home Visitation Program.

  • Identifies solutions for members aging out of foster care population and develops incentives to strengthen families.

  • Enhances quality approaches along the continuum of care.

  • Addresses gaps and identifies solutions in collaboration with the Medical Management team related to crisis service delivery including solutions to ED holds, SUDs, Reduction in out-of-state- placements and develops innovative solutions to serve difficult to place members.

  • Coordinates with internal departments to drive service development for special populations including adults with a history of foster placements and members with childhood trauma.

Required Qualifications

• Qualified candidates must reside in West Virginia.• Requires experience working as a Department of Human Services employee, and extensive understanding of the policies and procedures of that agency.• 5+ years experience in child welfare, through a state agency. • Must possess reliable transportation and be willing and able to travel state-wide up to 50% of the time. Mileage is reimbursed per our company expense reimbursement policy.• 2+ years’ experience with personal computer, keyboard navigation, and, Microsoft Outlook, Microsoft Teams, Microsoft Word and Dynamo or Electronic Medical Records (EMR).

• Bachelor's degree in behavioral health/mental health, counseling, social services or human services field required Preferred Qualifications

• Master's degree in behavioral health/mental health, counseling, social services or human services field preferred.• If RN, Nursing Diploma or Associate's degree required; BSN preferred• If licensed behavioral health clinician, Master's degree in social work, counseling, or marriage and family therapy required.

Education • Bachelor's degree in behavioral health/mental health, counseling, social services or human services field required• Master's degree in behavioral health/mental health, counseling, social services or human services field preferred.

Pay Range

The typical pay range for this role is:

$67,900.00 - $149,300.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. This position also includes an award target in the company’s equity award program.
