Tufts Medicine Advanced Practice Clinician - Cardiovascular Center Critical Care Float Pool in Boston, Massachusetts

Job Profile Summary

​This role focuses on providing professional and nonprofessional nursing care services in accordance with physician orders. In addition, this role focuses on performing the following Advanced Practice related duties: Under the direction of a physician, provides health care services to patients, ensuring optimal patient care. A professional individual contributor role that may direct the work of other lower level professionals or manage processes and programs. The majority of time is spent overseeing the design, implementation or delivery of processes, programs and policies using specialized knowledge and skills typically acquired through advanced education. A senior level role that requires advanced knowledge of job area typically obtained through advanced education and work experience. Typically responsible for: managing projects / processes, working independently with limited supervision, coaching and reviewing the work of lower level professionals, resolving difficult and sometimes complex problems.

Job Overview

This position services as the patient/family/ liaison for an assigned clinical program. Works closely with a broad spectrum of healthcare providers and administrative personnel to ensure optimal patient care and pre-treatment planning. Responsible for planning, and coordinating a plan of care for patients as part of the multidisciplinary team. This clinical provider will provide patient care within regulations in accordance to their licensure.

Job Description

Minimum Qualifications :

  1. Master’s degree in Nursing.

  2. Advanced Practice Registered Nurse (APRN) license.

  3. Specialty Certifications appropriate for the Program.

  4. Basic Life Support (BLS) certification.

  5. Eligible for state and federal controlled substances registration.

  6. Extensive clinical, project management, and leadership experience.

Preferred Qualifications :

  1. Multilingual.

Duties and Responsibilities : The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list. Other duties and responsibilities may be assigned.

  1. Provides clinical leadership and direction within assigned Program. Sets priorities for patient care

  2. Applies advanced nursing knowledge and experience to provide care to a assigned caseload of patients. Collaborates with the Attending Physician. This care involves comprehensive assessment, diagnosis and treatment of common problems encountered throughout the disease process.

  3. Provides comprehensive symptom management throughout the illness trajectory with specific attention to goals of care.

  4. Demonstrates the knowledge and skills necessary to provide care that is appropriate to the age of patients served in the clinical area.

  5. Collaborates and communicates with a wide range of multidisciplinary providers with the goal of achieving an exceptional patient experience.

  6. Identifies high risk patients and works with the appropriate services to achieve the best possible patient outcomes.

  7. Demonstrates advanced knowledge of pharmacology/medications in ordering and administering drugs and monitoring the patient’s response. Medication orders reflect this knowledge.

  8. Writes clear, comprehensive initial and follow up notes.

  9. Develops and maintains relationships between all members of the administrative and Clinical staff, thus producing high performing, patient focused teamwork.

Physical Requirements :

  1. Inpatient or outpatient clinic care areas.

  2. Continuous exposure to sick patients and bodily substances.

  3. Work requires frequent lifting of patients.

Skills & Abilities:

  1. Ability to care for chronically ill adult patients with a broad range of medical problems.

  2. Possess the maturity, self-confidence, and ability to follow complex patient management plans in collaboration with the attending surgeon.

  3. Possesses and applies the skills and knowledge necessary to provide care to patients throughout the life span, with consideration of aging processes, human development stages and cultural patterns in each step of the care process.

  4. Ability to provide primary care for a caseload of patients according to practice guidelines and hospital policies, procedures and protocols.

  5. Leads by example, Positive thinking, focused on goal.

  6. Communicates effectively through listening, persuasion, and influence.

  7. Plans, sets goals, analyzes results.

  8. Team building skills; Sets examples, provides vision delegates, positive, energetic

  9. Demonstrates flexibility, creativity, patience, and persistence.

  10. Able to handle situations of high stress and pressure.
