Job Summary: The Field Transitional Care Nurse (Clinical Liaison) is an integral member of the CVS Care Transitions Team. The Transitional Care Nurse (RN) will work closely with patients, health care providers and partners to facilitate timely post discharge support for patients transitioning from an acute care setting to home.

Essential Functions:

  • The Transitional Care Nurse will complete face-to-face visits with patients/caregivers during the hospital stay to assess post-discharge needs

  • At least 55% of your week is spent working in the field

  • Use the nursing process to provide telephonic case management support for 30-days post discharge

  • Maintain patient privacy in compliance with HIPAA, policies, and partnerships

  • Understands and interprets medical information within scope of practice

  • Completes all necessary documentation in Epic and other digital platforms

  • Monitor, schedule, and track patient appointments with care team providers

  • Completes other tasks as assigned

Required Qualifications

  • Bachelor’s Science Nursing

  • Active unrestricted RN license in the state of employment or multi-state compact license

  • Minimum of 5 years of experience as a Registered Nurse

  • Minimum of 2 years of acute care experience in a hospital setting as a Registered Nurse

  • Minimum of 1 year of case management experience as a Registered Nurse

  • Valid and current driver’s license and auto insurance

  • Ability to work from the field a minimum of 55% of your week

  • Meet immunization requirements of the assigned facilities

Preferred Qualifications

  • Knowledge of electronic health records

  • CPR certification

Minimum Knowledge, Skills & Abilities

  • Able to read, write, speak understand and satisfactorily communicate in English in person, over the phone and via email.

  • Proficient with MS Outlook, Word, PowerPoint, and Excel

Pay Range

The typical pay range for this role is:

$29.10 - $62.31

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
