Position description

Senior Services in Southeast MN is seeking a Nurse Practitioner or Physician Assistant to work as part of a multidisciplinary care team to provide care to Mayo Clinic patients living in skilled nursing facilities throughout the Owatonna, Austin & Albert Lea, MN area. Within in this position the NP/PA will work collaboratively with physicians to provide holistic care including acute, chronic, preventive and palliative care services. The provision of care includes but is not limited to comprehensive evaluation, ordering and interpreting diagnostic tests, prescription of pharmacologic agents and treatments and non-pharmacologic therapies, and counseling patients including advanced care planning. Ideal candidates have experience in Primary care, Geriatrics and Palliative Care. Travel will be required throughout the Southeast Minnesota region.

Qualifications

Masters prepared graduate of an accredited school of nursing and Nurse Practitioner program. Current Minnesota RN license and certification as an ACNP, ANP, GNP or FNP. (DNP is preferred)

Or graduate of an accredited Physician Assistant program with current PA certification by NCCPA and registration as a PA by the MN state Board of Medical Practice.

DEA registration and BLS certification required

Experience in primary care, palliative care geriatrics
