State of Minnesota Clinical Program Therapist 4 - Social Work Specialist Senior - Pod Leader in Anoka, Minnesota

Job Summary Job Summary *_This position works primarily weekdays, with occasional weekends and holidays required._ Anoka-Metro Regional Treatment Center (AMRTC) is a secure inpatient psychiatric hospital serving adults with mental illness in a large, campus-based setting. The population also includes individuals civilly committed for specialized care, such as competency for criminal court (Rule 20), emergency 48-hour bed holds, and co-occurring mental illness/chemical dependency. Located just 15 minutes north of the Twin Cities metro, this modern facility offers compassionate, person-centered care in a pleasant environment appreciated by patients and employees alike. The Social Work Pod Leader provides integrated health care and social work services to patients and their families to ensure the patient’s illness, recovery, and transition from one care setting to another are considered within the context of their unique biopsychosocial needs, individual and community safety needs, and the specific demands, risks and opportunities of the person's environment. The Social Work Pod Leader functions in a lead role to provide consultation, guidance, and assistance in disposition planning for patients and also provides individual and group therapy to individuals across the facility. Duties include: * Participate as an active member of the interdisciplinary team in the assessment, screening, and treatment plan formulation and implementation * Provide individual and/or group psychotherapy consistent with treatment plan and within the scope of practice; * Deliver psychosocial education and information for patients and their families; * Promote a timely return to the community through the development of a comprehensive discharge and aftercare plan, incorporating utilization management principles, risk management principles and continuity of care; * Provide clinical oversight to staff as assigned, as well as consultation to and collaboration with the treatment team. * Establish productive relationships and act as primary liaison with community providers who serve our patients, including county case managers, law enforcement, and other health, mental health and social service agencies. Qualifications Minimum Qualifications To qualify, applicants must: * Master's degree in social work, AND * Be licensed or eligible for licensure by the State of Minnesota as a Licensed Independent Clinical Social Worker (LICSW); AND * Have at least three (3) years of post-Master's degree experience providing social work practice in a behavioral health setting.
