Albany Medical Center Program Coordinator - Center of Excellence for Alzheimer's Disease in Albany, New York

We have an exciting opportunity to join our multidisciplinary team at the Center of Excellence for Alzheimer's Disease in the role of Program Coordinator to make a significant impact on advancing comprehensive specialty care for our dementia patients, to provide meaningful support to their caregivers and to advance education about the disease in the designated region

Under the general supervision of Director of Alzheimer’s Program, the Program Coordinator will oversee the day-to-day clinical and administrative operations of the NYS DOH grant funded Center of Excellence for Alzheimer’s Disease (CEAD). The Coordinator will oversee the outreach programs of the Center, ensuring the delivery of comprehensive education programs to the medical community in the designated region and development of strong collaborations with all related organizations and entities. The Coordinator will be responsible for collecting key data points for the mandatory quarterly reports and work plans, assisting the Program Director with the development of all applicable work plans and reports required by the Grant.

The Coordinator combines strong communication and team skills with the ability to focus on multiple priorities and drive successful completion of projects and tasks by established deadlines. The Coordinator is able to interact successfully with multidisciplinary teams including physicians, APPs, residents, social workers as well as clinical and administrative personnel. The Coordinator possesses strong leadership skills and the ability to drive change and assist the Program Director with implementing new initiatives in the Center and sustain collaborations and partnerships with the medical community in the region.

Specific Grant Related Responsibilities Include:

  • Coordinate the work of CEAD to build and nurture a strong, professional and passionate team in all areas of the Alzheimer’s Center at Albany Medical Center’s work

  • Provide operational leadership and expertise to staff.

  • Ensure that the Alzheimer’s Center at Albany Medical Center, the CEAD is consistently presented in a strong, positive image to relevant stakeholders, as well as maintain and enhance our reputation in the community.

  • Maintain a working knowledge of significant developments and trends in public health, with particular emphasis on Alzheimer’s disease prevention and control.

  • Investigate and interpret pre-award and post-award grant guidelines with all Center staff.

  • Consistently and systematically collect key data points on the goals and objectives established by the DOH grant to assist the Director in completion of all required work plans and reports.

Specific Responsibilities for Program Coordinator:

  • Develop and implement all systems necessary to ensure the orderly operation of the program, including but not limited to intake and assessment tools, policy/procedure/development/review, and database management.

  • Oversight of clinical operations of the division, including physician template development & management, residency continuity clinic oversight and prompt resolution of patient relation issues and bumped patient appointments.

  • Complete a variety of administrative tasks, reports and correspondence.

  • Seek opportunities to apply for various grants and take the lead in completing the applications.

  • Collaborate with a variety of related health/human service providers to produce information and educational programs and opportunities for networking.

  • Respond and present requests for educational/informational programs by civic organizations.

  • Meet with patients and/or families facing challenges because of their disease process. Provide care coordination and/or ongoing consultation as needed.

  • Counsel and refer families regarding SSD, Medicare, Medicaid, Managed Long Term Care Programs, TBI programs, etc.

  • Liaison with faculty and students of Albany Medical College in the education of Alzheimer’s disease.

  • Coordinator of the Generations Together Service Learning Program for AMC medical students.

Qualifications:

The ideal candidate will be experienced in handling a wide range of administrative and financial related tasks and will be able to work independently with little supervision. This person will be exceedingly well organized, flexible and must enjoy the administrative challenges of coordinating and supporting an office of diverse professionals and projects. The ability to interact with staff in a fast-paced, every-changing environment, often under pressure, while remaining flexible, proactive, resourceful and efficient, with a high level of professionalism and confidentiality is crucial to this role.

  • Master’s degree in Social Work

  • Minimum of 3 years of healthcare management experience

  • Minimum of 2 years’ experience in the field of neurodegenerative diseases.

  • Excellent time management and ability to proactively identify and manage multiple priorities.

  • In-depth knowledge of strategic planning, grant writing and development and budget management.

Salary Range: $28.99/hr - $46.39/hr
