Albany Medical Center Clinical Dietitian in Albany, New York

Under the direction of the Nurse Supervisor, the Clinical Dietician will be responsible for providing the following services to HIV infected clients:

• Plans and executes nutrition care/counseling via the Nutrition Care Process (NCP) to all HIV infected persons being treated at the Division of HIV Medicine (DHIVM), including but not limited to: Newly diagnosed HIV+, hospital follow-up, wasting syndrome, lipodystrophy, diabetes, renal failure, pregnancy, obesity, cancer, hyperlipidemia, CVD, HTN, anemias, gastrointestinal and liver disease, and other patients seen at the DHIVM.

• Performs comprehensive nutritional assessments in accordance with the NCP and develops nutrition care plan with patient

o Baseline nutrition assessments for all new patients

o Annual nutrition assessments

o Other nutrition assessments as requested by medical staff or patient

o Reassessments as needed.

• Utilizes motivational interviewing to facilitate patient movement toward goals

• Provides nutrition education and counseling in accordance with the patient’s NCP.

• Provides resources, as needed, to facilitate a healthy diet on a limited budget

• Provides Medical Nutrition Therapy to clients as requested by medical provider or recommended by RD.

• Provides engagement efforts and reengagement efforts to engage and retain patients in nutrition services

• Provides follow up within 1-2 weeks of nutrition assessments to reinforce goals and address barriers to meeting action steps

• Maintains caseload of patients receiving nutrition services

o Maintains quarterly contact with patients enrolled for nutrition services

• Coordinates all needs regarding nutrition supplementation:

o Initial nutrition assessment, six-month screen and annual reassessment to determine need and appropriate formula/dosage.

o Obtain prior authorization for Medicaid participants.

o Obtain prescription from provider.

o Coordinate with pharmacy for patient to receive supplement.

o Troubleshoot any issues between patient, pharmacy, and/or insurance plan, as necessary.

• Collaborates with multidisciplinary team and participates in case conferences, as needed

• Documents nutritional encounters in AIRS.

• Supervision of dietetic students as necessary

• Coordinates with external nutrition programs for AMC DHIVM patients

o Completes nutrition assessments as needed

o Facilitates referral, as needed

o Assures assessments are forwarded to community nutrition providers upon receipt of formal patient authorization

• Participates in eligibility determination and recertification process

EDUCATION and/or EXPERIENCE:

Bachelor's degree in nutrition or related field, Master's degree preferred, Registered Dietitian with the ADA, and 1 to 3 years documented experience in the provision of out-patient nutrition services to medically compromised patient population.

LANGUAGE SKILLS:

Ability to read and understand medical information, knowledge of medical terminology helpful. Fluency in written and oral English. Spanish is a plus. Ability to respond to routine inquiries or complaints from clients, regulatory agencies, or members of the community.
